Southside Community Health Services accepts self pay, private insurance, and MNsure products including Medical Assistance, Minnesota Care, and Qualified Health Plans (QHP). Also offers services on a sliding fee scale based on federal poverty guidelines of family size and income.

Hours:

 Monday - Friday, 7:00 am - 5:00 pm.
