Provides full service hospital and dental clinic for Native American population.
Provides Medical, Dental, Lab, Radiology, Public Health, WIC, Nutrition, Contract Care, Pharmacy with mail order, Behavioral Health, Optometry, Community Health Service and Mammography.
Hours: Monday, Tuesday, Wednesday, Thursday, Friday 7 AM - 5 PM
Intake procedure:Must complete application and create Chart
Fees: Accepts Medicaid, Medicare, Private Pay Insurances and offers sliding scale fees.
Eligibility: Must be member of a federally recognized Tribe. There are exceptions to this requirement. Contact agency for details.
Documents required: To open a chart and begin services must bring Social Security card; Certificate of Degree of Indian Blood (CDIB); Photo ID; or Letter of descendency issued by a federally recognized Tribe; Current insurance information (Medicaid, Medicare, SoonerCare or private insurance)
