The sliding fee scale is a tool All Care uses to determine how much patients are responsible for after receiving medical/dental care. The sliding fee scale is based on family size and income. 

Hours:

Monday - Thursday: 8:00am - 7:00pm.

Friday: 8:00am - 5:00pm.
